Upon retrieving the GECK, LW was exposed to an unhealthy amount of radiation, Butch and her packs have been in need of radaway for some time, but they irresponsibly forget to restock again before this mission. What starts as a small cough develops into an additional itchy dryness of her skin. Far from home and any doctor, Butch panics, unsure if his girlfriend is going to improve or die at any moment. She stays as positive as possible, reassuring him that she always makes it out ok.

This is the part where you can choose if you want her to die with him, or turn her into a ghoul over the course of a few days/weeks, giving butch the unimaginably difficult decision to attempt to ghoulify himself to be near-immortal as she (due to the fact that he’ll out age her over time) or die from the radiation sickness and not ghoul up, OR she’ll leave him so he won’t have to make the choice.

“Watch over Dogmeat while I’m gone.”
Those were the last words that his fellow vaultie had said to him before heading out into the wastes with her ghoul bodyguard, Charon. He had argued with her decision to ultimately make him stay behind and baby sit the dog, but between 
her stubborness and Charon’s gruff disposition he had ultimately resigned himself to his fate. Not that he didn’t like Dogmeat, no it was quite the contrary. Butch found that he was most comfortable when the flea bag was hanging around him, sitting there waiting for him to bend down and scratch the top of his head. He didn’t like admitting it but he found it easier to sleep when Dogmeat squeezed into bed with him. Maybe it was just because he felt less alone, maybe it was just because he had never really had a pet; but for whatever reason Butch was happy to have the dog by his side.
